Our urgent care and walk-in clinic is open 7 days a week and we are ready to perform your DOT Physicals. Come in to AFC Urgent Care from 8:00AM to 8:00PM Monday through Friday and 8:00AM to 5:00PM on Saturday & Sunday. No appointment necessary.

Going into your DOT/CDL Physical requires certain documentation. Please review and make sure to bring all listed items for your medical examiner to complete your Commercial Driver’s License (CDL) or Department of Transportation (DOT) physical.

A DOT physical exam is valid for up to 24 months. The medical examiner may also issue a medical examiner’s certificate for less than 24 months when it is desirable to monitor a condition, such as high blood pressure.

If the medical examiner finds that the person he/she examined is physically qualified to drive a commercial motor vehicle, the medical examiner will furnish one copy of the results to the person who was examined and complete a Medical Examiner’s Certificate. It is the patient’s responsibility to provide copies of this certificate to their employer.

Commercial Driver’s License (CDL) or Department of Transportation (DOT) physicals can only be done by providers who have completed training and testing and are listed in the National Registry of Certified Medical Examiners (NRCME). Here are some guidelines to help ensure a smooth DOT (re)certification process.

Things To Prepare For Your Appointment

  • Make sure to continue taking your prescribed medications as usual.
  • Wear loose clothing for the physical exam.
  • Don’t use the restroom just before your appointment. You will need to give a urine sample.

When you come to your DOT Physical Appointment you will need:

  1. Driver’s License
  2. Hearing aids (with extra batteries)
  3. Glasses (If you require correction to your vision, whether or not you wear contacts, you must bring a pair of glasses)
  4. Full bladder; be prepared to provide a urine sample.
  5. Complete Medication List from your pharmacy. (with dosage amounts included)

Drivers with the following Health Issues will need to complete specific forms:

Heart Issues, Diabetes or Pre-Diabetes, Hypertension or on hypertensive medication, Sleep Apnea or on CPAP machine, Respiratory Disorders, Smoker over age 35, Neurological Disorders, Mental/Behavioral Health issues, Blood Thinners, Vision, Controlled Substances.

Any patient operating a motor vehicle under a FMCSA-issued medical waiver must have the appropriate waiver form with them at the time of their physical examination. If you have other health problems or recent surgery, you may need to provide additional information. Failure to provide necessary documentation may mean we are not able to certify you for driving at the time you are seen. You may be asked to return at a later date at an additional charge to you.

If you fail to pass your exam due to misrepresentation or a disqualifying medical condition, you will still be responsible for paying for your DOT physical.
